Requirements

  • Confirmed Registration Card/Bill from hospital
  • Completed MHSS Application form.
  • Valid means of identification.
  • Letter of introduction
  • Letter of set-off
  • Letter of undertaking
  • Clean CRB report
  • Copy of Marriage Certificate in situations where a husband is the primary obligor.
  • In the case of 3rd party the Sponsor/Guarantor, the following additional documentation will be required:

    1.    Irrevocable domiciliation of Sponsor/Guarantor Salary
    2.    Letter of Lien/Set Off
    3.    Personal Guarantee backed by a statement of net worth
